Home Health LPN Job Description

About the Role:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:

  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and prevention.
  • Conduct thorough assessments, identify patient needs, and collaborate with the healthcare team to ensure comprehensive care.
  • Administer medications, treatments, and therapies as prescribed by physicians, and document patient progress and outcomes.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and transitions.
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes, adhering to regulatory requirements and CenterWell Home Health standards.

Requirements:

  • Graduate of an accredited Licensed Practical Nursing program or vocational nursing school.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
