Home Health LPN

At CenterWell Home Health, we're dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Home Health LPN, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
Key Responsibilities:- Provide professional nursing services under the direction of a RN, adhering to state Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies.
- Deliver skilled nursing interventions to address patients' rehabilitative needs, preventative care, and treatment of illnesses.
- Collaborate with RN Supervisor to modify the Plan of Treatment as needed and communicate with physicians regarding verbal orders.
- Utilize a systematic, individualized approach to implement the nursing plan of care, ensuring timely communication with patients, physicians, and healthcare teams.
- Maintain confidentiality and adhere to professional standards in sharing patient information.
- Prepare and submit documentation as required by the Company and/or facility, ensuring accurate visit/shift notes.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
