Job Summary

We are seeking a highly skilled and compassionate Home Health Visiting Nurse to join our team at Home Health Advantage INC. in the South Chicago area. As a field nurse, you will provide high-quality nursing care to patients in their homes, working closely with our team to deliver exceptional patient outcomes.

Responsibilities
  • Implement and manage individualized care plans for patients, ensuring their needs are met and exceeded.
  • Provide skilled nursing services, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Maintain accurate and detailed records of patient interactions, treatments, and progress.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Graduate of an accredited nursing program (LPN/RN) and current licensure in the state of Illinois.
  • Minimum one year of experience in home health care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering exceptional patient care.
