As a Home Health LPN, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
Key Responsibilities:- Provide professional nursing services under the direction of a RN, adhering to state Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies.
- Deliver skilled nursing interventions to address patients' rehabilitative needs, preventative care, and treatment of illnesses.
- Utilize a holistic approach to provide problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
- Collaborate with RN Supervisors to ensure seamless care coordination and make necessary changes to the Plan of Treatment.
- Communicate effectively with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
- Maintain confidentiality and adhere to professional standards in sharing patient information.
- Prepare and submit documentation as required by the Company and/or facility, ensuring accurate and timely visit/shift notes.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
