Key Responsibilities

The Senior Geriatric Care Nurse Manager plays a vital role at AHF by applying advanced nursing skills and a thorough understanding of the Ryan White Program to assist clients in their designated region. This position involves close collaboration with patients aged 60 and above who are living with HIV/AIDS and face various psychosocial and clinical health challenges.

Your responsibilities will include:

  • Providing health education and medical coordination.
  • Conducting follow-ups on medical treatments and advocating for patients.
  • Performing Comprehensive Assessments and developing personalized Care Plans.
  • Delivering ongoing education to ensure patients comprehend their health conditions and medications.
  • Offering emotional support and advocacy to mitigate health disparities.

As a Senior Geriatric Care Nurse Manager, you will enhance AHF's reputation by embodying core nursing principles. You will engage with patients to identify their strengths and areas for improvement, assisting them in setting realistic health goals. Your collaborative approach will help bring the essence of the Ryan White Care Act to our most vulnerable populations.

By establishing Needs Assessments, Care Plans, and necessary referrals, your contributions will directly support AHF's objectives in fulfilling the requirements of our Ryan White contract. Additionally, your efforts will align with the standards set by the HIV/AIDS Bureau and the Department of Health and Human Services.

Your role will involve assisting patients in maintaining healthcare appointments, adhering to prescribed treatments, and coordinating care for any comorbid or chronic conditions. Ultimately, you will support clients in achieving their health goals through education, counseling, and increased access to services within the continuum of HIV Health Services.

Licenses and Certifications: A Bachelor's degree in Nursing (BSN) is required, along with a minimum of one year of Case Management experience. Familiarity with Ryan White Services is preferred. Certification from the Association of Nurses in AIDS Care (ANAC) is a plus.
