YOUR RESPONSIBILITIES


In the role of Geriatric RN Medical Care Manager, you will engage with patients aged 60 and above who are living with HIV/AIDS.

Your duties will encompass delivering health education, coordinating medical services, and advocating for patients. You will perform Comprehensive Assessments, create individualized Care Plans, and provide continuous support aimed at enhancing overall health outcomes.

QUALIFICATIONS

  • Bachelor’s degree in Nursing (BSN)
  • At least 1 year of experience in Case Management
  • Familiarity with Ryan White Services is preferred
  • Certification from the Association of Nurses in AIDS Care (ANAC) is preferred
