Key Responsibilities- Patient Care: Provide primary and specialty care for geriatric patients, including diagnosis, treatment, and management of acute and chronic conditions.
- Assessment: Conduct thorough physical examinations, assess patient medical histories, and develop individualized care plans.
- Medication Management: Prescribe and manage medications, ensuring appropriate dosages and monitoring for potential side effects or interactions.
- Coordination of Care: Collaborate with multidisciplinary teams, including nurses, social workers, and other healthcare professionals, to ensure comprehensive and coordinated patient care.
- Family Communication: Communicate effectively with patients and their families regarding diagnosis, treatment plans, and ongoing care needs.
- Health Promotion: Promote healthy aging practices, preventive care, and lifestyle modifications to enhance the well-being of elderly patients.
- Education: Educate patients, families, and caregivers on managing age-related conditions and maintaining independence.
- Documentation: Maintain accurate and detailed medical records, ensuring compliance with all regulatory requirements.
- Quality Improvement: Participate in quality improvement initiatives and contribute to the development of policies and procedures aimed at enhancing geriatric care.
- Education: Medical degree (MD or DO) from an accredited institution.
- Residency: Completion of a residency program in Internal Medicine or Family Medicine.
- Fellowship: Completion of a fellowship in Geriatric Medicine.
- Licensure: Valid medical license to practice in TX
- Certification: BE/BC
