YOUR ROLE IN OUR MISSION
As a Senior Geriatric Care Nurse Manager, you will leverage your robust nursing skills and knowledge of the Ryan White Program to support clients in your designated area.
This role involves collaborating closely with patients aged 60 and above who are living with HIV/AIDS and face various psychosocial and clinical health challenges. Your responsibilities will encompass health education, medical coordination, treatment follow-ups, and patient advocacy.
Working within our Healthcare Center, you will conduct Comprehensive Assessments, formulate Care Plans, and provide ongoing education to ensure patients comprehend their diagnoses and medications, while striving to meet adherence goals aimed at enhancing overall health through counseling and skill development.
You will also provide advocacy and emotional support to clients to mitigate health disparities. Additionally, you will facilitate or refer patients to licensed medical providers for evaluations related to activities of daily living (ADLs), cognitive abilities, vision, hearing, nutrition, fall prevention, osteoporosis, and medication interactions.
As a Senior Geriatric Care Nurse Manager, you will uphold AHF's esteemed reputation by applying strong foundational nursing principles. You will engage with patients at their level, identify their strengths and areas for improvement, and assist them in setting realistic health goals.
Your collaboration and engagement skills will be vital in bringing the essence of the Ryan White Care Act to our most vulnerable populations.
By conducting Needs Assessments, creating Care Plans, making referrals, and providing social support, your contributions will directly impact AHF's success in fulfilling the objectives of our Ryan White contract. Beyond contractual obligations, your efforts will align with the measures set forth by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

In this position, you will work as part of a dedicated team to guarantee that patients receive timely access to comprehensive medical care and social services while promoting independence and self-sufficiency.
Furthermore, you will assist patients in scheduling and attending healthcare appointments, adhering to antiretroviral therapies (ARTs) and other prescribed medications, conducting home visits, and communicating with healthcare providers to coordinate care for comorbid or chronic conditions.
Ultimately, you will support clients in achieving adherence goals designed to enhance their overall health through education, counseling, skill-building, and improved access to services within the continuum of HIV Health Services in a primary medical care setting.
QUALIFICATIONS:
A Bachelor's degree in Nursing (BSN) along with a minimum of one year of Case Management experience is required. Familiarity with Ryan White Services is preferred. Certification from the Association of Nurses in AIDS Care (ANAC) is also desirable.
