Case Manager ICMS Housing Scattered Sites Specialist

The Case Manager ICMS Housing Scattered Sites position at St. Joseph Center is a vital role in ensuring the delivery of quality care to our clients. As a team member with social service professionals and paraprofessionals, you will work together to provide comprehensive case management services, adhering to established rules and regulations, and upholding the mission of St. Joseph Center.
Key Responsibilities- Provide high-quality case management services to clients, serving as their central point of contact throughout the transition from homelessness to community living.
- Evaluate individual and family needs for permanent housing, working with the housing program manager and other case management staff to identify suitable options.
- Assist clients in establishing eligibility for subsidized housing, collecting required documents, and completing applications.
- Develop professional relationships with clients, maintaining a caseload of scattered site participants, and conducting regular face-to-face case management sessions.
- Prepare forms and documentation for initial housing voucher approval and annual recertification with the Housing Authority.
- Conduct on-site office hours at assigned properties, initial home visits, and ongoing home visits as needed.
- Provide ongoing monthly case management sessions to monitor clients' progress, offer community referrals, and advocate on their behalf when necessary.
- Offer education on successful housing retention, social isolation prevention, and relapse risk reduction, including setting up and transporting clients on field trips.
- Support clients in developing independent living skills, including money management, budgeting, housekeeping, nutrition, and social/recreational activities.
- Develop and implement individualized service plans, emphasizing coordinated strategies for addressing short and long-term housing needs, financial stability, and other risk factors.
- Assist clients with maintaining medication and treatment regimens, accompanying them to appointments with health, mental health, and other care providers.
- Utilize a harm reduction, client-focused, and strength-based model in all interactions.
- Ensure timely entry of case notes into the Homeless Management Information System (HMIS) and CHAMP, submitting notes for review by the program manager weekly.
- Manage client file organization, updating forms as required, and responding to urgent requests for assistance from clients or landlords.
- Provide new resident orientation as needed and develop close working relationships with collaborating agencies, including property management and landlords.
- Participate in meetings with collaborative agencies and other community meetings as directed.
To be successful in this role, you must have:
- Bilingual fluency in Spanish language preferred.
- High motivation and self-starting ability.
- Excellent organizational skills and the capability to work in a fast-paced environment.
- Strong knowledge of housing market, housing programs, and resources.
- Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
- One to two years of experience providing services to highly vulnerable populations.
- Bachelor's degree from an accredited college or university in social work, human services, or a related field.
- Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed.
