Job Description

The Case Manager plays a vital role in ensuring the delivery of quality care to residents living in Project Based housing. As a team member, they work closely with social service professionals and paraprofessionals to adhere to established rules and regulations, as well as the mission of St. Joseph Center.

Key Responsibilities
  • Provide high-quality on-site case management services to residents.
  • Serve as the central point of contact for clients navigating the transition from homelessness to tenant living.
  • Evaluate individual/families for permanent housing in conjunction with the housing program manager and other case management staff.
  • Assist individuals/families to establish eligibility for subsidized housing, including document collection and application completion.
  • Develop professional relationships with clients and maintain a caseload of project-based participants, conducting regular face-to-face case management sessions.
  • Assist with the preparation of forms and documentation required for initial housing voucher approval with the Housing Authority.
  • Conduct regular on-site office hours at assigned properties.
  • Conduct initial home visits upon new tenant lease-up and ongoing quarterly home visits, or more often as determined by need.
  • Conduct ongoing monthly case management sessions to monitor clients' progress and provide community referrals.
  • Provide education on topics related to successful housing retention, social isolation prevention, and relapse risks.
  • Support clients in developing independent living skills, including money management, housekeeping, nutrition, and social/recreational activities.
  • Develop and implement individualized service plans, emphasizing coordinated strategies for addressing short and long-term housing needs.
  • Assist clients with maintaining medication and treatment regimens, including accompanying/transporting clients to appointments.
  • Utilize a harm reduction, client-focused, and strengths-based model.
  • Ensure case notes are entered into the Homeless Management Information System within 48 hours of completing direct case management services.
  • Manage client file organization and ensure appropriate forms are updated as required.
  • Respond to urgent requests for assistance from clients or landlords.
  • Provide new resident orientation as required.
  • Develop close working relationships with collaborating agencies, including property management and landlords.
  • Participate in meetings with collaborative agencies and other community meetings as directed.
Requirements
  • Bilingual fluency in Spanish language preferred.
  • Highly motivated and self-starter.
  • Ability to communicate with and relate to a diverse group of people.
  • Excellent organizational skills and capability to work in a fast-paced environment.
  • Strong knowledge of housing market, housing programs, and resources.
  • Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • One to two years of experience providing services to highly vulnerable populations.
  • Bachelor's degree from an accredited college or university in social work, human services, or a related field.
  • Valid California Class C Driver License or ability to utilize an alternative method of transportation.
