Job Description:- We are seeking a skilled Clinical Care Coordinator to join our team at McLeod Health.
- As a Clinical Care Coordinator, you will oversee the process of care delivered to patients, work collaboratively with the healthcare team, and provide leadership to ensure quality patient outcomes.
- You will examine records for medical treatments and interventions to avoid payment denial and optimize reimbursement.
- The role includes patient assessment for discharge planning, level of care determination, and coordination of care progression throughout the continuum.
- Bachelor's degree in Nursing required.
- 1-2 years of recent acute care hospital experience preferred.
- Data entry/computer experience essential.
- Licensed RN to practice within the applicable region of employment required.
$60,000 - $80,000 per annum.
