We are seeking a skilled Hospice Nurse Homecare to join our team at McLeod Health. This is a PRN opportunity that requires active patient contact within the past year.

Job Summary

This role involves maintaining a professional image and providing exceptional customer service to patients, visitors, physicians, and coworkers. You will assess patients' needs based on initial evaluation, nursing assessment, and diagnostic data, developing a plan of care consistent with the physician's prescribed medical plan.

You will serve as a case manager and interdisciplinary leader for assigned patients, coordinating care with other team members. Additionally, you will recognize signs of active dying patients and communicate this information to the team.

Requirements

  • Active South Carolina RN License.">
  • At least one year of nursing experience.">
  • Current BLS certification.">
  • Ability to work with a team and maintain 24/7 RN availability.">
  • Participation in continuing education annually, including mandatory training.">

Estimated Salary: $65,000 - $85,000 per year
