Professional Care Coordinator

About the Role:
As a Professional Care Coordinator at McLeod Health, you will play a vital part in maintaining a professional image and delivering exceptional customer relations to patients, visitors, physicians, and coworkers.
Job Responsibilities:
- Coordinate patient care progression throughout the continuum of care
- Ensure high-quality, efficient, and cost-effective services are provided
- Meet directly with patients and families to assess needs and develop individualized continuing care plans
- Monitor length of stay and ancillary resource use
- Communicate information to the Utilization Management team for level of care determination and reimbursement
- Identify issues affecting quality of care, risk management, patient satisfaction, and physician opportunities
- Communicate critical elements of the plan of care to patients, families, and healthcare teams
- Cover various populations across the organization, including the Emergency Department
Qualifications:
- 1-2 years of recent acute care hospital experience required
- Data entry/computer experience required
- Registered nurse from an NLN accredited school of nursing required
- Licensed RN to practice within the applicable region of employment and coverage area of responsibility is required

Salary: $70,000 - $90,000 per year, depending on experience
