Salary:$85,000 - $110,000 per year
Job Description:
As a Care Coordinator RN, you will play a critical role in ensuring that patients receive the best possible care throughout their healthcare journey. Your responsibilities will include:
- Coordinating patient care progression throughout the continuum of care
- Assessing patient needs and developing individualized continuing care plans
- Communicating with healthcare teams to ensure seamless care delivery
Required Skills and Qualifications:
To be successful in this role, you will need:
- 1-2 years of recent acute care hospital experience
- Data entry/computer experience
- A registered nurse license from an NLN-accredited school of nursing
