Nurse Care Coordinator

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Patient Care Specialist. As a key member of our interdisciplinary care team, you will be responsible for coordinating and facilitating the care of patients in their homes, Community-Based Residential Facilities (CBRF), assisted living locations, hospitals, or other settings.
Key Responsibilities- Care Coordination: Assess and coordinate patient discharge planning needs with the healthcare team, ensuring a seamless transition of care.
- Skilled Nursing: Provide skilled nursing care within the established plan of care, including vital sign monitoring, wound care, IV management, and physical assessments.
- Communication: Collaborate with patients, families, and the healthcare team to develop a comprehensive plan of care, addressing unique patient needs and concerns.
- Documentation: Accurately and thoroughly document visit notes, treatments, and care, adhering to regulatory requirements.
- Education: Graduate of an accredited school of nursing or equivalent education for licensure.
- Experience: No experience required, but prior experience in home health or hospice care is preferred.
