Geriatric Care Coordinator

Manchester Center for Rehabilitation and Healing is seeking a skilled MDS Coordinator to join our team. The ideal candidate will possess a passion for geriatric care and have a strong understanding of the Minimum Data Set (MDS) process.
Key Responsibilities:- Oversee the accurate and thorough completion of the MDS, Care Area Assessments (CAAs), and Care Plans.
- Act as an in-house Case Manager, demonstrating detailed knowledge of residents' health status and critical thinking skills to develop an appropriate care pathway.
- Proactively communicate with the Administrator and Director of Nursing to identify regulatory risk and effectiveness of Facility/Community Systems.
- Capture the RUG score, which reflects the care and services provided.
- Ensure timely electronic submission of all Minimum Data Sets to the state database.
- Licensed Nurse with current, active license in the state of practice.
- Minimum two (2) years of clinical experience in a healthcare setting.
- Minimum of one (1) year of experience in a long-term care setting.
