Nurse Care Coordinator

Job Summary:
We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Care Coordinator. In this role, you will provide high-quality, patient-centered care to individuals and their families in the comfort of their own homes.
Responsibilities:
- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in on-call duties and provide after-hours support as needed.
Requirements:
- Current Registered Nurse licensure in the state of practice.
- Experience in hospice care or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
