This is a full-time field-based role.
Employees must be flexible to work within our 8-hour shift schedules during standard business hours, which typically run from 8:00 AM to 5:00 PM, including a one-hour lunch break. Occasional overtime may be required based on business needs.
In this position, you will conduct approximately 2-3 visits to healthcare facilities each day, supporting our mission from a remote setting.
Key Responsibilities:- Collaborate with the Assigned Patient Coordinator (APC) to ensure comprehensive member care coordination.
- Engage as a vital member of a collaborative healthcare team.
Data Management and Member Support:
- Maintain accurate member charts and perform data entry for:
- New members
- Post-hospital visit follow-ups
- Pre-visit updates
- Health maintenance tracking
- Quality metrics
- Lab results and medication updates
- Secure medical records post-hospitalization and after physician appointments
- Gather consult notes and discharge summaries
- Monitor updates for members receiving therapy
- Input lab results and medication changes into the Electronic Medical Record (EMR)
- Assist in scheduling member appointments and coordinating transportation.
- Facilitate educational sessions for facilities and members.
- Build and maintain relationships with healthcare facilities.
- Support virtual family communications and clinical visits by managing technology during sessions.
Assisted Living and Skilled Nursing Facility Tasks:
- Track skilled days and assist with documentation as directed by the APC.
- Help members utilize their benefits effectively.
- Coordinate transitions to skilled nursing facilities, including arranging transportation and gathering necessary medical documentation.
- Facilitate home health and durable medical equipment coordination.
Qualifications:
- High School Diploma or GED, or equivalent experience.
- Minimum of 18 months of clinical or medical record experience.
- Proficiency in EMR systems and Microsoft Office Suite (Word, Excel, Outlook).
- Understanding of medical terminology and experience with healthcare team collaboration.
- Reliable transportation for travel to client sites within designated areas.
- Willingness to travel up to 100% of the time.
- Ability to work full-time with flexible scheduling.
Preferred Qualifications:
- Experience with CMS compliance.
- Certification as a Nursing Assistant or Medical Assistant.
