Position Objective:

The PATH (Projects for Assistance in Transition from Homelessness) Case Manager is tasked with assisting individuals experiencing homelessness within the PATH initiative to secure stable housing and access necessary benefits. This role involves conducting thorough assessments, crafting personalized service plans, and tracking client progress. The case manager engages in weekly community-based case management to help clients achieve and sustain housing and benefits while enhancing their self-determination through effective illness management strategies. Collaboration with community organizations and mental health service teams is essential, along with participation in regular client reviews and program management meetings to ensure compliance with performance metrics and grant obligations.

Qualifications:

  • Bachelor's Degree in a health-related discipline, with a preference for candidates with two years of experience in mental health.
  • Possession of a valid driver's license and dependable transportation.

Key Skills:

  • Proven education, training, or experience in mental health services and/or co-occurring disorders.
  • Strong written and computer skills for timely and accurate documentation.
  • Exceptional organizational abilities and computer proficiency.
  • Self-motivated individual capable of working independently, collaboratively, and within community settings.
  • Experience in providing individual, family, and group counseling, as well as case management.
  • Ability to educate and support clients, families, and community resources.
  • Effective communication skills with team members.

Core Responsibilities:

  • Deliver mental health rehabilitation services and support to individuals and families, aiming to reduce hospitalizations and crisis situations while enhancing community integration for clients to achieve their recovery goals.
  • Ensure continuity of program services in an ethical, legal, and compassionate manner within a safe and therapeutic environment, adhering to all relevant regulatory and accreditation standards.
  • Actively participate as a team member in providing mental health services to clients and their families, integrating recovery principles into their daily lives.
  • Offer case management services, crisis intervention, skills training, outreach, and group services.
  • Complete documentation within designated time frames.
  • Attend scheduled meetings.
  • Contribute to quality improvement initiatives.
  • Perform additional duties as required to meet the needs of individuals facing severe mental health challenges and homelessness.
