Job Summary

We seek an experienced Licensed Practical Nurse to join our team as a Care Coordinator, providing critical support to patients and their families throughout the healthcare journey.

Responsibilities and Expectations:

  • Deliver high-quality patient care and education, focusing on disease state management and treatment adherence.
  • Build and maintain strong relationships with patients, caregivers, and healthcare providers to ensure seamless care coordination.
  • Maintain accurate records and reports to meet regulatory requirements and program objectives.
  • Participate in continuous quality improvement initiatives to enhance patient outcomes and service delivery.

Requirements and Qualifications:

  • Current licensure as a Licensed Practical Nurse in Tennessee.
  • Minimum 2 years of nursing experience, preferably in a similar role.
  • Excellent communication and interpersonal skills.
  • Able to work in a fast-paced environment and prioritize multiple tasks effectively.
  • Familiarity with electronic health records (EHRs) and medical software.

Compensation: $65,000 - $75,000 per year
