Job Summary

Join our dynamic team at Christ Community Health Services as a Patient Support Liaison In this role, you will play a vital part in reducing social barriers to health and improving patient outcomes for high-risk mental health patients.

About the Role

You will have a strong background in social work or a related field, with experience in patient care and social work. Your excellent communication skills and ability to work with diverse populations will enable you to build strong relationships with patients, families, and healthcare teams.

Your Responsibilities

  • Connect patients with community resources and support services to address their unique needs and circumstances.
  • Closely coordinate care with medical providers, BHCs, and other care team members to ensure seamless transitions and optimal patient outcomes.
  • Monitor patient progress and adjust interventions as needed to ensure effective support and care coordination.
  • Develop and maintain relationships with mental health and social service partners in the community to ensure coordinated services for patients.

Estimated Salary Range: $75,000 - $95,000 per year
