Salary

We believe in compensating our employees fairly, with an estimated salary range of $60,000-$80,000 per year, considering total compensation packages, skill sets, training, education, and organizational needs.

Job DescriptionJob Summary

The Medical Case Manager will provide direct case management services to supported clients, bridging services in the continuum of HIV care. This role plays a crucial part in ensuring clients have access to appropriate support services, including treatment adherence counseling, linkage to healthcare, psychosocial and mental health support, insurance coverage, educational resources, and stable housing.

  • Client Engagement and Follow-up: Provide an overview of case management services, develop a patient-centered care plan, and ensure client achieves viral suppression within one year of program intake.
  • Data Entry: Document client enrollment in Ryan White Eligibility System and other relevant programs, providing accurate, complete, and timely data entry and reporting.
  • Case Notes and Documentation: Maintain accurate records of client-specific activities, including progress made towards goals, and establish eligibility for health insurance or medical coverage.
Responsibilities

This position requires regular and predictable attendance, participation in weekly coaching sessions, and completion of Performance Reviews and SMART Goals. Additional duties may include traveling to meet staff, clients, or community partners at FFA locations or other locations.

Required Skills and Qualifications

To perform this job successfully, an individual must possess:

  • Bachelor's degree preferred.
  • 1-3 years of experience in customer service, education, human service delivery, community outreach, public health, benefits enrollment, or related fields.
  • Excellent organization and time management skills.
  • Cultural sensitivity and ability to work respectfully and inclusively with a diverse population.
  • Strong verbal and written communications skills.
