Job Summary:As a Social Worker at St. Gabriel's Hospice, you will provide emotional support, counseling, and practical assistance to patients and families facing serious illness or end-of-life care. You will conduct comprehensive psychosocial assessments, collaborate with interdisciplinary teams, and develop personalized care plans.
Responsibilities:- Conduct thorough assessments to understand patients' needs, preferences, and goals.
- Collaborate with healthcare teams to create tailored care plans.
- Provide emotional support, counseling, and crisis intervention to patients and families.
- Assist with advance care planning, including discussions about end-of-life wishes.
- Connect patients and families with community resources, financial assistance, and support groups.
- Advocate for patients' rights and ensure their voices are heard.
- Maintain accurate and confidential records.
- Master's degree in Social Work (MSW) from an accredited program preferred.
- Licensed as required by the state where practicing. Current drivers' license.
- One year of experience in healthcare, hospice, or related settings preferred.
- Strong communication, empathy, and active listening skills.
- Knowledge of end-of-life care, grief, and bereavement support.
- Ability to work collaboratively in a multidisciplinary team.
