Key Responsibilities:- Explain hospice services and Medicare benefits to patients and families, obtaining informed consent and election of benefits documents as needed.
- Submit timely and accurate documentation and paperwork to facilities, adhering to company policies.
- Participate in the development and implementation of patient care plans, collaborating with the interdisciplinary team to ensure comprehensive and coordinated care.
- Provide psycho-social support and counseling services to patients and families, addressing concerns related to financial means, insurance, living arrangements, caregiver stressors, and community resource needs.
- Maintain respect for patients' environments and belief systems, upholding nondiscriminatory practices regarding age, race, religion, sex, sexual orientation, national origin, physical or mental disability, and other personal matters.
- Minimum of a Bachelor's degree in Social Work, Psychology, Sociology, or a related field, with one year of social work experience in a healthcare setting.
- Licensure required to practice social work, as per state regulations.
- Strong written and verbal communication skills, with the ability to articulate and embrace the Compassus philosophy.
The demands of this role require a team member to effectively perform essential functions, with adaptations made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
