About the Role

We are seeking a highly skilled and compassionate Client Care Coordinator to join our team at Homes for the Homeless. As a key member of our organization, you will play a vital role in providing mental health and related services to homeless families.

Key Responsibilities
  • Conduct comprehensive bio-psychosocial assessments with each family to understand their strengths and service needs.
  • Monitor and lead social service and counseling support, including timely interventions and referrals to community-based services.
  • Ensure effective social services delivery, integrating models and best practices such as trauma-informed and family-centered care.
  • Document client progress toward service plan goals and ensure families receive full support in shelter.
  • Support the Family Services department by working alongside the Case Manager and collaborating with staff to address challenging client situations.
  • Improve linkages to mental health, education, and community-based services, strengthening permanency outcomes for families with children in shelter.
  • Manage record keeping, data collection, and evaluation on the effectiveness of services for families.
  • Assist with unit inspections and support the Family Services department by conducting trainings.
Requirements
  • Masters degree in Social Work or Mental Health Counseling.
  • Licensed Master Social Worker (LMSW) or Licensed Master Health Counselor (LMHC) in the state of New York.
  • Familiarity with substance use disorders, addiction treatment services, domestic violence, child abuse and maltreatment, trauma, and mental health conditions.
  • Sensitivity and awareness of working with homeless and vulnerable populations.
  • Bilingual skills are a plus.
