About the Role

CAMBA is seeking a highly skilled and compassionate Client Care Coordinator to join our team at the Park Avenue Family Residence. As a key member of our staff, you will play a critical role in providing comprehensive support services to homeless families.

Key Responsibilities
  • Assist clients in completing intake applications, screenings, and forms, ensuring accurate and timely documentation.
  • Conduct initial risk assessments of clients and their families, identifying potential areas of concern and developing strategies for support.
  • Prepare and maintain client files, ensuring all necessary documentation is up-to-date and easily accessible.
  • Provide instant response services to clients in crisis, ensuring their immediate well-being and safety.
  • Collaborate with clients and case managers to develop and implement Independent Living Plans (ILPs), setting short-term and long-term goals for client success.
  • Identify and connect clients with community resources, including medical services, psychiatric services, and other essential support services.
  • Follow up with clients and referral organizations to ensure timely and effective service delivery.
  • Consult with internal and external stakeholders to identify causes of client problems and develop effective solutions.
  • Act as an advocate for clients, arranging for necessary medical, psychiatric, and other tests and examinations to inform service delivery.
  • Facilitate case conferences with stakeholders to ensure a collaborative approach to services.
  • Provide regular progress updates and reports to ensure accountability and continuous improvement.
Requirements
  • Licensed Master of Social Work (LMSW) required.
  • Must sign NYC DOH/MH attestation stating that candidate has not been the subject of a founded child abuse report.
