Key Responsibilities- Assist clients in completing intake applications, screenings, and forms, ensuring accurate and timely documentation.
- Conduct initial risk assessments of clients and their families, identifying potential areas of concern and developing strategies for support.
- Prepare and maintain client files, ensuring all necessary documentation is up-to-date and easily accessible.
- Provide instant response services to clients in crisis, ensuring their immediate well-being and safety.
- Collaborate with clients and case managers to develop and implement Independent Living Plans (ILPs), setting short-term and long-term goals for client success.
- Identify and connect clients with community resources, including medical services, psychiatric services, and other essential support services.
- Follow up with clients and referral organizations to ensure timely and effective service delivery.
- Consult with internal and external stakeholders to identify causes of client problems and develop effective solutions.
- Act as an advocate for clients, arranging for necessary medical, psychiatric, and other tests and examinations to inform service delivery.
- Facilitate case conferences with stakeholders to ensure a collaborative approach to services.
- Provide regular progress updates and reports to ensure accountability and continuous improvement.
- Licensed Master of Social Work (LMSW) required.
- Must sign NYC DOH/MH attestation stating that candidate has not been the subject of a founded child abuse report.
- Competitive salary range: $58,000-$62,000 annually.
- Comprehensive benefits package, including health insurance, dental insurance, 403(b) retirement plan with employer match, paid time off, and paid holidays.
