Job Summary

The Rehabilitation Nurse Case Manager plays a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimal outcomes. This position requires a skilled and compassionate nurse to provide, coordinate, and direct care specific to the needs of each rehabilitation patient.

Key Responsibilities
  • Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
  • Current State of Alaska Registered Nurse License.
  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification.
  • Associates Degree in Nursing required; BSN preferred.
  • One year of Rehab or Med/Surg experience preferred.
  • Experience in Utilization Review, Discharge planning, or Case Management preferred.
