Key Responsibilities:- Coordinate the transplant process, including pre-transplant evaluation, transplant surgery, and post-transplant care.
- Develop and implement individualized plans of care for patients undergoing transplant.
- Collaborate with multidisciplinary teams to ensure seamless communication and coordination of care.
- Provide education and support to patients and their families throughout the transplant process.
- Participate in performance improvement activities to enhance patient outcomes and quality of care.
- Bachelor of Science in Nursing from an accredited nursing program.
- 2 years of relevant experience in transplant nursing or a related field.
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
- American Heart Association (AHA) Health Care Provider BLS certification.
