Advanced Practice Provider

We are seeking an experienced Advanced Practice Provider to join our dynamic interdisciplinary team in the Heart Failure and Transplant Program at the University of Virginia. The successful candidate will be responsible for providing high-quality patient care, collaborating with physicians and other healthcare professionals, and contributing to the growth and development of the program.
Key Responsibilities- Provide primary outpatient management for patients with Left Ventricular Assist Devices (LVADs), including medication management, scheduling, and coordination of appointments.
- Collaborate with physicians to maintain patient care and develop treatment plans.
- Conduct LVAD device interrogations, RAMP studies, and perform wound care/wound management for driveline exit sites.
- Participate in decision-making regarding patient eligibility for advanced therapies.
- Coordinate VAD outpatient visits, testing, procedures, and readmissions.
- Serve as an on-site liaison to other departments and outside facilities.
- Provide LVAD education to patients, families, and staff.
- Participate in regular programmatic meetings, including patient selection committee, journal club, quality and process improvement, and administrative meetings.
- Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree.
- License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia.
- Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.
The job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease.
