Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our Transplant Team. As a key member of our multidisciplinary team, you will be responsible for providing exceptional patient care and coordination of transplant services.

Key Responsibilities
  • Provide relationship-based care to patients and families, focusing on population management and individualized plans of care.
  • Utilize advanced critical thinking to develop, implement, and evaluate care plans, ensuring optimal patient outcomes throughout the continuum of care.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and effective communication.
  • Provide after-hours and weekend coverage, serving as the primary point of contact for pre- and post-transplant patients and internal and referring providers.
  • Coordinate business and after-hours care, including telephone assessments, complex care coordination, and treatment based on comprehensive assessments and strong interpersonal communication skills.
Requirements
  • Bachelor of Science in Nursing from an accredited nursing program.
  • 2 years of relevant experience.
  • Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
  • American Heart Association (AHA) Health Care Provider BLS certification.
  • American Board of Transplant Coordinator Certification at time of hire or within 2 years of employment.
Physical Demands

The job requires standing for prolonged periods, frequent travel, bending/stooping, and the ability to lift/push/pull 20-50lbs. May be exposed to chemicals, blood/body fluids, and infectious disease.
