Job Summary

We are seeking a highly skilled and experienced Transplant Nurse Specialist to join our team at the Commonwealth of Virginia. As a key member of our transplant team, you will be responsible for providing expert care to patients undergoing transplant procedures.

Key Responsibilities
  • Provide relationship-based care to patients and families, focusing on population management and collaborative multidisciplinary team work.
  • Develop, implement, and evaluate individualized plans of care that educate, empower, and optimize patient outcomes throughout all transitions within the continuum of care.
  • Assess patients and families, utilizing strong clinical knowledge and interpersonal communication skills to provide telephone assessments, complex care coordination, and treatment based on comprehensive assessments.
  • Collaborate with multidisciplinary team members to ensure timely and effective communication of clinical decisions and patient responses to interventions.
  • Participate in performance improvement activities, quality assurance, and patient safety initiatives to maintain compliance with regulatory standards.
  • Stay up-to-date with the latest transplant nursing practices, research, and technologies to provide evidence-based care and implement innovative solutions.
Requirements
  • Bachelor of Science in Nursing from an accredited nursing program.
  • 2 years of relevant experience in transplant nursing or a related field.
  • Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
  • American Heart Association (AHA) Health Care Provider BLS certification.
  • American Board of Transplant Coordinator Certification at time of hire or within 2 years of employment.
Physical Demands

The job requires standing for prolonged periods, frequent travel, bending, and stooping. The ability to lift, push, and pull 20-50lbs is also required. May be exposed to chemicals, blood/body fluids, and infectious disease.
