Critical Illness Care Coordination

At Select Specialty Hospital, we seek a dedicated and compassionate Clinical Liaison Specialist to serve as our advocate for patients during the pre-admission process. This role requires a strong clinical background, problem-solving skills, and the ability to work autonomously and collaboratively as part of a team. Key responsibilities include performing clinical assessments, counseling with patient families, and partnering with hospital case managers to create smooth patient transfer processes.

Responsibilities
  1. Collaborate with clinical teams to evaluate patients' statuses and medical needs.
  2. Develop results-producing relationships with hospitals and healthcare professionals.
  3. Perform clinical assessments to identify patients' medical needs.
  4. Counsel with patient families to provide emotional support.
  5. Partner with hospital case managers to plan the best continuum of care.

Requirements
Qualified candidates must be registered nurses or respiratory therapists with a strong clinical background, problem-solving skills, and the ability to work autonomously and collaboratively as part of a team. Salary is a salaried position with periodic on-call responsibility.
