Your role will involve educating patients and families, coordinating care, and collaborating with the healthcare team to deliver high-quality care that meets defined standards.

Key Responsibilities:

* Prepare patients for office visits by reviewing their history, collecting relevant clinical information, and documenting in the medical record.
* Complete Point of Entry (POE) and address care gaps during patient encounters.
* Prepare rooms and equipment for procedures and assist practitioners as needed.
* Initiate interventions and document responses to treatment according to established workflows and guidelines.
* Participate in emergency care as directed by practitioners or registered nurses.
* Educate patients and provide preparation materials for diagnostic studies according to guidelines.
* Schedule appointments for patients with lab, radiology, or specialty consultations as ordered by practitioners.
* Monitor electronic messaging and emails related to patient or provider requests and manage messages within the scope of practice.
* Participate in departmental or regional quality improvement activities and initiatives.
* Engage patients, families, and caregivers in the coordination of care.
* Resolve patient concerns promptly.
* Provide departmental orientation and training to new team members on standard work, policies, and procedures.
* Participate in the supervision of unlicensed personnel in the delivery of patient care.
* Perform other duties as assigned.

Requirements:

* Graduate of an accredited nursing (LPN/LVN) program.
* Practical Nurse License (Georgia) required at hire.
* Basic Life Support required at hire from the American Heart Association.

Preferred Qualifications:

* Minimum six (6) months of healthcare experience.
* Customer service aptitude demonstrated through a Customer Service Assessment.
