Job Summary

About The Job

The successful candidate will be responsible for coordinating the facility's Resident Assessment Instrument (RAI) process, ensuring accurate completion of all MDS assessments, and implementing comprehensive plans of care. You will work closely with our healthcare team to identify residents' needs and develop tailored care plans that promote their physical, emotional, and social well-being.

In addition to your clinical expertise, you will possess excellent communication and interpersonal skills, enabling you to effectively collaborate with colleagues, physicians, and family members to achieve optimal patient outcomes. Your attention to detail and ability to prioritize tasks will be essential in meeting deadlines and managing competing demands in a fast-paced environment.

We offer a competitive salary range of $45-50 per hour, depending on experience, as well as a generous benefits package including 80 hours of paid vacation, sick leave, direct deposit, online time clock and payroll system, on-line scheduling system, 403b retirement plan, insurance after 60 days of eligibility, voluntary vision/FSA, and employee assistance program. RAIC certification is preferred but not required. Pre-employment drug screening and background check conducted prior to start.
