Lifespan Corporation seeks a Medical Care Coordinator to join our team. This role is responsible for providing high-quality patient care and support under the supervision of a Registered Nurse.

About the Role

This position requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. The ideal candidate will be able to collaborate with healthcare professionals to provide comprehensive care to patients and families.

Responsibilities
  • Collaborate with Registered Nurses to organize work and set priorities.
  • Perform personal care skills, basic nursing assistant skills, and rehabilitation skills as outlined by the Rhode Island Department of Health Professional Licensing and Facility Regulation.
  • Recognize and report signs and symptoms, and maintain a safe environment for patients.
  • Document data accurately in the EHR and demonstrate ability to perform responsibilities in emergency situations.
Requirements

To be successful in this role, you will need:

  • A current Rhode Island license as a Nursing Assistant.
  • High school graduate or equivalent preferred.
  • Ability to perform basic computer skills.
  • Certified in American Heart Association Healthcare Provider Basic Cardiac Life Support (BLS).
