Palliative Nurse Practitioner Opportunity

Constellation Health Services is a renowned healthcare organization committed to delivering exceptional patient experiences and high-quality outcomes.

We are seeking an experienced Palliative Care Nurse Practitioner (NP) to join our dedicated team of healthcare professionals.

Estimated Salary: $120,000 - $150,000 per year

This role involves providing comprehensive care to patients in their homes and various facilities in the area.

Key Responsibilities:

  • Conduct thorough assessments and develop patient care plans in collaboration with relevant disciplines.
  • Monitor and report on patients' physical and emotional status to nursing supervisors and attending physicians.
  • Administer medications and treatments as prescribed by physicians for patients in their home setting.
  • Participate in weekly Interdisciplinary Team Meetings to share updates on patients' progress.
  • Maintain accurate documentation according to agency standards.

Required Qualifications:

  • A Master's degree, post-masters certificate, or doctoral degree in nursing from a university accredited by CCNE or NLNAC.
  • A current license to practice nursing in New Hampshire and/or Maine.
  • Current National Board Certification from ANCC or AANP as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP).
  • Experience in hospice or palliative care, clinical oncology, geriatrics, intensive care, or community and family health.
