Palliative Nursing Specialist

As a Palliative Care Nurse Practitioner at Constellation Health Services, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Estimated Salary: $130,000 - $160,000 per year
This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Key Responsibilities:
- Conduct comprehensive assessments and develop patient care plans in collaboration with relevant disciplines.
- Monitor and report on patients' physical and emotional status to nursing supervisors and attending physicians.
- Administer medications and treatments as prescribed by physicians for patients in their home setting.
- Participate in weekly Interdisciplinary Team Meetings to share updates on patients' progress.
