Job Description
We are seeking an Advanced Practice Nurse Practitioner (NP) to join our palliative/hospice program. This individual will work in patients' homes and facilities, performing comprehensive assessments, coordinating care with disciplines, administering medications, evaluating patient status, and participating in Interdisciplinary Team Meetings.
This role requires Master's degree in nursing, current license to practice nursing, National Board Certification, and experience in hospice or palliative care. A Palliative care certification or hospice certification is preferred.
