JOB TITLE: Coordinator of Patient and Family Support Services at Commonwealth of Virginia

The Coordinator of Patient and Family Support Services is instrumental in tackling food insecurity and improving the overall health of patients within the Commonwealth of Virginia community. This role encompasses the management of resources and supplies, collaboration with wellness professionals, and facilitation of client intake procedures. Reporting to the Project Manager and Manager of Clinical Administration, this position guarantees access to vital resources, encourages wellness, and cultivates a nurturing environment for healing and recovery.

Duties & Responsibilities:

Food Security Initiatives
  • Act as a primary contact for data collection and food distribution for patients facing food insecurity.
  • Engage with the community to identify needs and promote awareness of available resources.
  • Establish and maintain partnerships with local organizations and businesses to bolster food security efforts.
  • Foster relationships with staff and community members to enhance food security initiatives.
  • Utilize partnerships to expand the reach and effectiveness of food security programs.
  • Assist in overseeing food resource distribution, including logistics coordination and inventory management.
  • Support licensed staff in educational programs focusing on nutrition and culinary skills.
  • Review training materials with individuals and communities on sustainable food security practices.
  • Assist in identifying funding opportunities through grant writing and fundraising initiatives.
  • Oversee and train volunteers involved in food distribution and educational programs.
  • Ensure a rewarding and productive experience for volunteers.
The Wellness Space Role
  • Respond to inquiries, provide information about services, and address guest concerns.
  • Ensure guests feel comfortable and have a positive experience during their visit.
  • Maintain all equipment and supplies in optimal condition.
  • Support wellness practitioners by preparing spaces and assisting with client intake.
  • Provide assistance during group sessions, both virtual and in-person.
  • Offer information about various healing modalities and services available.
  • Collect and analyze feedback to identify areas for improvement and enhance customer satisfaction.
  • Collaborate with team members and wellness practitioners to foster a positive work environment.
Patient and Family Support Services
  • Work with the Patient and Family Support Services team and leadership to manage inventory of patient resource materials.
  • Assist in maintaining visible displays and inventory of resources in the support center.
  • Ensure the preparation and distribution of approved patient resource materials.
  • Manage inventory of new patient packets and resource guides.
  • Respond to requests from patients, families, and clinical staff for resource materials.
  • Assist in overseeing volunteers and support special projects as needed.
  • Participate in quality improvement initiatives as appropriate.
  • Listen to and anticipate customer needs, providing clear explanations and prompt assistance.
EDUCATION & EXPERIENCE:
  • Bachelor's degree in Business Administration, Project Management, Healthcare Administration, or a related field is required.
  • A minimum of three years of relevant work experience is required.
  • Proficiency in computer skills, including MS Office and general computer applications, is required.
  • Strong English communication skills (both verbal and written), organizational abilities, time management, attention to detail, and multitasking skills are essential.
