Responsibilities

  • Provide Support and Resources: Assist patients in accessing community resources for nutritional, emotional, transportation, and housing support, empowering them to make informed decisions about their care.
  • Advocate for Patients: Advocate and assist patients with accessing healthcare and other benefits, ensuring they receive the support they need to thrive.
  • Educate and Empower: Educate patients and their families regarding end-stage renal disease treatment options, including dialysis, transplant, and medical management, helping them make informed decisions about their care.
  • Coordinate Care: Coordinate translation services for patients and caregivers, ensuring seamless communication and support.
  • Develop Individualized Care Plans: Collaborate with the interdisciplinary care team to develop and implement individualized care plans based on psychosocial assessments, helping patients achieve their rehabilitation goals.
  • Collaborate with the Care Team: Work closely with the interdisciplinary care team to assess ongoing needs and modify care plans as necessary, ensuring patients receive comprehensive and coordinated care.
