Job Title: Registered Nurse - Hospice Care

Job Summary:

Brighton Hospice is seeking a compassionate and skilled Registered Nurse to join our team. As a Hospice Nurse, you will provide high-quality care to patients and their families, focusing on comfort, dignity, and quality of life.

Responsibilities:

  • Conduct initial and ongoing assessments of patients' physical, functional, spiritual, and emotional needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide education and support to patients and their families on end-of-life care, self-care techniques, and disease management.
  • Communicate effectively with the medical director, clinicians, and other healthcare professionals to ensure seamless care coordination.
  • Document patient progress, observations, and services provided in a timely and accurate manner.

Requirements:

  • Active RN license and valid driver's license.
  • Strong verbal and written communication skills.
  • Ability to work independently and as part of a team.
  • Passion for providing compassionate care and making a difference in patients' lives.
