Position: Case Manager

Key Responsibilities:

1. Conduct thorough interviews and assessments for applicants.
2. Regularly meet with clients and their families in various settings, including their homes and community locations.
3. Provide crisis intervention, informal counseling, and advocacy services to enhance health and safety.
4. Offer support for complex dual diagnosis issues and related behaviors.
5. Assist clients with budgeting, credit repair, benefit acquisition, healthcare access, court-related matters, medication monitoring, parenting education, and connections to educational, employment, and volunteer opportunities.
6. Schedule and accompany clients to important meetings and service-related appointments.
7. Teach essential living skills, social skills, and healthy coping mechanisms.
8. Develop, monitor, and implement Individual Service Plans (ISP).
9. Ensure compliance with housing leases and maintain health and safety standards.
10. Organize and maintain accurate client documentation, including files and program reports.
11. Enter detailed meeting notes and assessments into online databases.
12. Organize and facilitate peer support groups.
13. Act as a liaison with landlords, property management, and housing agencies.
14. Assist clients in maintaining apartments to meet quality standards by addressing maintenance and sanitary issues.
15. Provide support for client housing relocation as necessary.
16. Serve as a coordinator within each client's network of service providers.
17. Conduct outreach to community partners to promote services and recruit applicants.
18. Build and maintain positive relationships with human service and government agencies.
19. Attend supervision, staff meetings, and community-based gatherings.
20. Conduct supervised urine drug screenings for clients regularly.
21. Provide limited after-hours on-call support.
22. Demonstrate strong written and verbal communication skills.
23. Adhere to organizational policies, rules, and codes of conduct.
24. Perform other related duties as assigned.

Qualifications:

- High school diploma required.
- Valid driver's license and reliable transportation necessary.
- Minimum of one year of relevant experience in homeless services, mental health, and substance abuse resources.
- Proficient in computer skills, including Microsoft Word, Excel, and other web-based applications.
- Must be flexible, reliable, and possess a positive attitude.
- Excellent time management skills with the ability to meet deadlines.
- Strong organizational skills with the ability to multitask and complete tasks effectively.
- Ability to work independently as well as collaboratively in a team environment.
