Position: Case Manager

Key Responsibilities:

1. Conduct thorough interviews and assessments of applicants.
2. Regularly meet with clients and their families in various settings, including their homes and community locations.
3. Provide crisis intervention, informal counseling, and advocacy to enhance health and safety.
4. Offer support for complex issues related to dual diagnoses and associated behaviors.
5. Assist clients with budgeting, credit repair, benefit acquisition, healthcare access, and court-related matters.
6. Facilitate parenting education and connect clients to educational, employment, and volunteer opportunities.
7. Schedule and accompany clients to necessary meetings and appointments.
8. Teach essential life skills, social skills, and effective coping mechanisms.
9. Develop, monitor, and implement Individual Service Plans (ISP).
10. Ensure compliance with housing leases and maintain health and safety standards.
11. Organize and maintain accurate client documentation and program reports.
12. Input detailed meeting notes and assessments into online databases.
13. Organize and lead peer support groups.
14. Act as a liaison with landlords, property management, and housing agencies.
15. Assist clients in maintaining their apartments to meet quality standards.
16. Support clients with housing relocation as needed.
17. Coordinate services among each client's network of providers.
18. Engage with community partners to promote services and recruit clients.
19. Build positive relationships with human service and government agencies.
20. Attend supervision, staff meetings, and community-based gatherings.
21. Conduct supervised drug screenings for clients regularly.
22. Provide limited after-hours on-call support.
23. Exhibit strong written and verbal communication skills.
24. Adhere to organizational policies and code of conduct.
25. Perform additional related duties as assigned.

Qualifications:

- High school diploma required.
- Valid driver's license and reliable transportation necessary.
- Minimum of one year of relevant experience in homeless services, mental health, and substance abuse.
- Proficient in computer applications, including Microsoft Word and Excel.
- Must be flexible, reliable, and possess a positive attitude.
- Excellent time management and organizational skills are essential.
- Ability to work independently and collaboratively within a team environment.
