5. Assisting clients with budgeting, credit repair, benefit acquisition, healthcare access, and court-related matters.
6. Monitoring medication adherence and providing parenting education.
7. Connecting clients to educational, employment, and volunteer opportunities, as well as mainstream services.
8. Scheduling and accompanying clients to important meetings and appointments.
9. Teaching essential life skills, social skills, and coping mechanisms.
10. Developing, monitoring, and implementing Individual Service Plans (ISP).
11. Ensuring compliance with housing leases and maintaining health and safety standards.
12. Organizing and maintaining accurate client records and program reports.
13. Entering detailed meeting notes and assessments into online databases.
14. Facilitating peer support groups and organizing relevant curriculum.
15. Acting as a liaison to landlords and housing agencies.
16. Assisting clients in maintaining their living spaces to meet quality standards.
17. Supporting clients with housing relocation as necessary.
18. Coordinating services among various providers within each client's network.
19. Conducting outreach to community partners to promote services and recruit applicants.
20. Building positive relationships with human service and government agencies.
21. Participating in supervision, staff meetings, and community-based discussions.
22. Performing supervised drug screenings for clients.
23. Providing limited after-hours on-call support.
24. Demonstrating strong written and verbal communication skills.
25. Adhering to organizational policies and codes of conduct.
26. Performing additional related duties as assigned.
Qualifications:
- High school diploma required.
- Valid driver's license and reliable transportation necessary.
- Minimum of one year of relevant experience in services related to homelessness, mental health, and substance abuse.
- Proficient in computer applications, including Microsoft Word and Excel.
- Must be flexible, reliable, and possess a positive attitude.
- Excellent time management skills and ability to meet deadlines.
- Strong organizational skills with the ability to multitask and follow through on tasks.
- Ability to work independently as well as collaboratively in a team environment.
