Job Overview
The Advanced Practice Clinician (NP/PA) aligns with our medical team, managing a distinct patient population. Key responsibilities include providing outpatient and inpatient care, facilitating discharges and outpatient follow-up, and adhering to clinical pathways and protocols.
Key Accountabilities
- Assessment and Planning
- Conducts comprehensive history and physical examinations for acute and complex chronically ill patients.
- Orders and collects relevant diagnostic information and procedures.
- Initiates interventions and resuscitative actions as needed.
- Identifies and mitigates risks associated with patient care, including medication side effects and fluid imbalance.
- Collaboration and Communication
- Maintains open communication with the interdisciplinary healthcare team.
- Facilitates care coordination with patients, families, and staff.
- Documentation and Quality
- Documents patient progress in daily progress notes, transfer summaries, discharge summaries, and clinic notes.
- Ensures timely, compliant, and accurate documentation.
Requirements
Certifications: Licensed Nurse Practitioner - Tennessee, Physician's Assistant Certified - Tennessee
Work Experience: Relevant work experience, 1 year
Education: Master's
