About the Role

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.

Responsibilities
  • Conduct thorough health assessments, reviewing medical history, medication adherence, social environment, and other risks.
  • Collaborate with physicians, case managers, social workers, and other healthcare professionals to provide comprehensive care.
  • Develop and implement personalized care plans to address patients' unique needs.
  • Provide education and support to patients and their families to promote healthy behaviors and disease management.
Requirements
  • Master's degree in Nursing or related field.
  • Current RN and NP licensure in the state of practice, with prescription authority.
  • Board certification in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health records.
  • Excellent communication and interpersonal skills.
