Job Summary:

Covenant Health (Tennessee) is seeking a skilled Nurse Practitioner to join our team in Palliative Care. As a key member of our healthcare team, you will provide primary care to patients under the supervision of a physician preceptor, following established medical protocols.

Responsibilities:

Maintain established departmental policies and procedures, objectives, quality assurance program, and safety standards.

Provide direct patient care, evaluate outcomes, consult with other specialists as required, and adjust processes as indicated to ensure optimal patient care.

Collaborate with physician preceptor to formulate and review medical protocols.

Obtain complete medical history of the patient.

Identify health hazards in the patient's environment as well as patient risk factors. Identify and consider the cultural patterns, economic status, and patient resources in plan of care.

Perform appropriate physical exam as indicated.

Obtain appropriate laboratory specimens.

Analyze objective and subjective data for appropriate diagnosis and treatment.

Assure treatment and/or plan of care is consistent with established protocols.

Monitor and ensure accuracy of recording on patient medical records and maintains all required reports, records, statistics, etc. Documents all pertinent information accurately, legibly, and in a timely manner.

Refer patient to appropriate services which exceed scope of practice of the practitioner.

Provides health promotion counseling and develops patient teaching information.

Serves as a resource person for other staff personnel. May supervise clinical staff.

Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

Attends meetings as required and participates on committees as directed.

Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.

Qualifications:

Minimum Education: None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

Minimum Experience: One (1) year of experience.

Licensure Requirement: Current DEA license. Must have and maintain certification by the American Nurse Association (ANA) or American Academy of Nurse Practitioners (AANP) with specialty applicable to the area of responsibility – Family Nurse Practitioner, Adult-Gerontology, Psychiatric-Mental Health. Must meet state licensure requirements. CPR required.
