Home Health Nurse Practitioner

As a Home Health Nurse Practitioner with Covenant Health, you will provide primary care to patients in the comfort of their own homes. You will work under the supervision of a physician preceptor, following established medical protocols to ensure optimal patient care.
Responsibilities:
Maintain established departmental policies and procedures, objectives, quality assurance program, and safety standards.
Provide direct patient care, evaluate outcomes, consult with other specialists as required, and adjust processes as indicated to ensure optimal patient care.
Collaborate with physician preceptor to formulate and review medical protocols.
Obtain complete medical history of the patient.
Identify health hazards in the patient's environment as well as patient risk factors. Identify and consider the cultural patterns, economic status, and patient resources in plan of care.
Perform appropriate physical exam as indicated.
Obtain appropriate laboratory specimens.
Analyze objective and subjective data for appropriate diagnosis and treatment.
Assure treatment and/or plan of care is consistent with established protocols.
Monitor and ensure accuracy of recording on patient medical records and maintains all required reports, records, statistics, etc. Documents all pertinent information accurately, legibly, and in a timely manner.
Refer patient to appropriate services which exceed scope of practice of the practitioner.
Provide health promotion counseling and develops patient teaching information.
Serves as a resource person for other staff personnel. May supervise clinical staff.
Maintain professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
Attends meetings as required and participates on committees as directed.
Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
Performs other duties as assigned.
Qualifications:
Current DEA license.
Must have and maintain certification by the American Nurse Association (ANA) or American Academy of Nurse Practitioners (AANP) with specialty applicable to the area of responsibility – Family Nurse Practitioner, Adult-Gerontology, Psychiatric-Mental Health.
Must meet state licensure requirements. CPR required. Employee must have a valid Tennessee driver's license and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
