Job Description

This role involves creating and managing a plan of care for individual patients. You should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

Key Responsibilities:
  1. Assess patient needs and develop personalized care plans
  2. Collaborate with other healthcare professionals to implement care plans
  3. Maintain accurate records of patient care and communicate with supervisors
  4. Transport completed client charts to clients' homes within 7 days of chart preparation
Qualifications:
  • Currently licensed Registered Nurse (RN) in state of employment
  • Previous experience as a Registered Nurse is preferred
  • Excellent time management and communication skills
  • Valid driver's license and first aid/CPR certification required
Compensation:

The estimated salary range for this role is $85,000 - $110,000 per year, depending on location and experience.
