Atrium Health is seeking a talented Patient Care Coordinator to join our team. As a patient care coordinator, you will be responsible for ensuring that patients receive the highest level of care in the pre-hospital setting at customer venues. You will work closely with other healthcare professionals to coordinate care delivery and promote health and a safe environment.

Responsibilities
  • You will collect data pertinent to the healthcare consumer's health or the situation and analyze it to determine the diagnoses or issues.
  • You will identify expected outcomes for a plan individualized to the healthcare consumer or the situation and develop a plan to achieve them.
  • You will implement the identified plan, coordinate care delivery, and employ strategies to promote health and a safe environment.
  • You will evaluate progress toward attainment of outcomes and make adjustments as needed.
Requirements

The ideal candidate will have a diploma in nursing and a current RN license or temporary license as a Registered Nurse Petitioner in the state where they work and reside. They should also have at least three years of experience in ED or Critical Care Environment and be certified in ACLS. The estimated salary for this position is $80,000 per year.
